Edition 2008/10 · PDF format

Used to report auto insurance claims. Includes vehicle information, accident details, driver and witness information, injury descriptions, and police report details.

What is ACORD 2?

ACORD 2 (Automobile Loss Notice) is the standard form for reporting an auto accident or auto claim to a carrier. It captures vehicle information, driver, accident details, injuries, witnesses, and police report data.

Brokers and insureds submit ACORD 2 to open the claim file. The carrier then assigns an adjuster, secures statements, inspects vehicles, and proceeds with claim handling.

Who needs this form

Anyone reporting an auto loss (fender bender, major collision, theft, vandalism) to their insurance carrier.

When to use it

Report immediately after an accident or theft. State law and policy provisions typically require prompt notice, generally within days.

  • Driver information
  • Other party information (vehicle, driver, insurance)

Common Mistakes
